If you supply me with your email address, I will search my database of valid Users
and try to locate this email address. If I find any matching User.Ids, I will email
your User.ID and Password to this email address.
Once your information is submitted and we find any matching User IDs, you should
receive an email with your User.ID and Password within a few minutes. If you have
NOT received your User.ID and Password by the next day and you think it should have
